Dear
When in admin part you click on image, you will see "Image options'
Please fill image title and description
Next please go to "Gallery settings" -> Fancy Box and set show "Show Image Title:" and "Show Image Description"

In free version of Image gallery for Joomla ordasoft.com/os-image-gallery.html you can't change "Fancy box" settings
