How can I add extra fields on the Lend Request Information page, where the requester enters their details? I need to add 'Phone' and 'Ecclesia'. I do not need the 'Postal Mail Address' field, so is it possible to turn that off?

You may rebuild Book Library Lend request with help code edit.
You need edit files:
{yours site}/components/com_booklibrary/booklibrary.html.php
{yours site}/components/com_booklibrary/booklibrary.php

But for add new fields you also will need add so fields in dataBase and rebuild work with Lend request.
