BookLibary Component Support

Hello there. First of all, I love this component! It's a great choice for my classroom library. I love the fact that you have a lend request for the books. It's perfect for what I need. However, I don't need all of the input fields that are available. I would like to delete the following input fields and related text.
email
your postal mailing address

All I would like to have is the "YOUR FULL NAME" field and the "LENT FROM" and "BOOK RETURN DATE".

Is this possible?
